Learn about how members, ages 3-19, can earn a $25 gift card.
It is important for your child to get regular well-child checkups. These visits are also called Early and Periodic Screening, Diagnostic and Treatment (EPSDT) visits or well exams. They are complete health checks for members under 21 years old.
The Back-to-School incentive offers a $25 gift card to Banner – University Family Care members ages 3–19 years, who attend their first EPSDT / Well-Child visit of 2023, between June 5 and September 5.
