This is an insurance benefit and is free for those with the included plans. The Dial Into Diabetes program is a voluntary, no-cost service to help you with managing your health.This program provides personalized phone appointments and typically spans a duration of three months. A team of diabetes experts (Certified Diabetes Care and Education Specialists, Dietitians, Nurses, Exercise Physiologists, Health Coaches, and Pharmacists) will co-create a diabetes action plan with you by phone. Our goal is to assist you in feeling good today and lowering your risk for complications in the future!
Upon registration, you should expect a phone call from our team within a week to discuss the next steps.
