Welcome to New Provider Orientation

Thank you for your interest in applying for medical staff at Banner Churchill Community Hospital. We hope you will find our resources useful and informative.

All new members of Banner Churchill Community Hospital medical staff must review the Banner Health system and facility-specific web pages.

Completion of both the Banner Health system and the Banner Churchill orientation module is required prior to approval of privileges and can be completed anytime while your medical staff application is in process. Please review and become with the resources below as well as:

Identification Badges Required on campus 

Call Medical Staff Services at (775) 867-7481 to complete your ID Verification and to obtain your ID Badge. You will need to provide a legible government issued ID card (current driver’s license, military ID or passport). Physicians, Advanced Practitioners, Allied Health practitioners, Residents and students must wear a Banner identification badge while on campus and should be worn above the waist. 

Parking

Provider parking is free, and is available on the East Side of the building, in the north parking lot nearest the 3 story side of the building. The first row of the smaller parking lot is for Physicians and Practitioners only. All other spots with white lines are for all staff members and available for use as well. Due to Covid, the employee entrance is badge in only, and Medical Staff Services will meet you at the employee entrance door to let you into the facility on your first day.

Physician Lounge 

The physician lounge is located on the first floor past the main lobby, to the left when heading to the clinic side of the building.  There is a pin pad access, and Medical Staff Services can provide you the code upon arrival. Snack items and drinks are readily available in the lounge, and lunch service is from 11:30 to 1pm with a daily menu or grill menu are available.

Security

Banner wants everyone to be safe on campus. Our Security patrols Monday through Thursday 6 am to 2 am, and Friday through Sunday from noon to midnight. This offers both internal and external support to providers, patients, associates and visitors. Their office is located in the Emergency Department lobby.

If you observe suspicious conduct or behavior while on hospital property or if you or someone else is the victim of a crime, call security dispatch at (775) 867-7022.

Medical Staff Services provides administrative support to the medical staff and facilitates communication related to medical staff matters. 

Responsibilities include assisting with regulatory compliance, credentialing, peer review, resident/student rotations, continuing medical education, committees, and maintaining current governing documents of the Medical Staff and Allied Health Staff: Bylaws, Policies, Rules and Regulations. 

Medical Staff Services is located on the second floor of the MOB, in the alcove directly across from the elevator bay and is open Tuesdays through Thursdays, from 8 a.m. to 4:30 p.m, unless otherwise scheduled. Staff is available Monday through Friday from 8 a.m. to 4:30 p.m. by phone or Teams.

Banner Churchill utilizes Cerner for electronic medical record documentation. Medical Records, or Health Information Management Services (HIMS), can be reached directly at (775)867-7040.

  • Dictation/Transcription: dial 13700 while in house or (833)494-2500 when not on campus. To dictate once you have called the system:
    • Enter your 5-digit ID (MedSeries 4 #) followed by #
    • Enter 2-digit facility code: 90 followed by #
    • Enter the work type + #
    • Enter the patient ACCOUNT number + # (If account number not known, DO NOT USE ZEROES, Enter three 9’s + # (999#)
    • You will be prompted to begin dictating
    •  Press 5 to end dictation and begin another dictation;
    • New Functions have been added:
      • Pressing * will play back all options while in Record/Playback mode
      • Pressing 0 will play back the values entered at the prompts(acct #, Work type, location)
      • Please contact the facility HIMS department to obtain new copies of the dictation instructions.

Clinical Documentation Specialists:

Compliant Documentation Management Program (CDMP) assists the physician, via the documentation clarification and coding process, to provide the most accurate and compliant documentation for each patient.

Banner Lassen Medical Center provides Clinical Documentation Specialists (CDSs) to assist physicians by providing the most up-to-date clinical knowledge and Medicare requirements to all documentation options. CDSs also can provide physicians with the impact these options can have on severity ratings.
CDSs can be utilized concurrently on the floors and will assist the physicians in using compliant language in addition to helping with communication to the hospital coders.

We want to make it as easy as possible for you to admit your patients to Banner Churchill Community Hospital.

A physician order is required for all patient admissions to Banner Churchill Community Hospital.

Patients may be treated only by providers who have submitted proper credentials and have been duly appointed to membership on the medical staff.

Admitting: 
Hours of operation: 
Monday through Friday from 6:30 a.m.- 5:30 p.m.
Saturday from 8 a.m. - 11 a.m.
Admitting after hours: call (775) 427-3298

Direct admits:
The attending/admitting physician or designee shall call Bed Placement at (775) 481-2741 with the provisional diagnosis prior to their patient’s admission. Providers will be required to submit the following information: 

  • patient name 
  • birth date 
  • sex 
  • face sheet
  • contact numbers for attending physician or his/her representative

In certain situations insurance information is necessary to ensure prior authorization is received.  A patient placement representative will arrange for bed assignment if a bed is available or will place the patient on a waiting list. If a direct admit is in advance of a procedure, the placement representative keeps the information until the procedure date, but providers should keep in mind there may be delays due to patient census.

Discharging
Discharge planning begins upon admission with an assessment of the patient’s condition and living environment and a review of anticipated needs at the time of discharge. 

The referral process allows for physicians, staff, patients, and families to ask for assistance. Communication and collaboration are an important aspect of the discharge plan.

Hospitalists 
Banner Churchill Community Hospital provides in-house hospitalist coverage for patients. Hospitalists provide acute inpatient care 24/7, assisting providers on admission and discharge, as well as coordinate care for inpatients during their hospital stay.

Clinical Performance Assessment & Improvement (CPAI) oversees the Performance Improvement Program, ensures Banner Churchill Community Hospital is compliant with accrediting and regulatory organizations.

The department utilizes department-specific and CPAI departmental data to implement evidenced-based solutions to improve specific departmental and overall hospital quality. The department proactively implements Banner franchise model/best practices and hospital wide solutions in anticipation of quality and risk issues that may impact patient care.

CPAI increases visibility of Banner Churchill Community Hospital’s positive quality outcomes both internally and externally to achieve quality recognition and implements solutions to ensure compliance with the National Patient Safety Goals.

Banner Churchill Community Hospital offers both on-site medical interpreters and the Cyracom interpretation system to make sure all patients and physicians can communicate effectively and safely.

It is Medical Interpreter Policy to ONLY use the hospital Medical Interpreter, video remote interpreter or telephonic interpreter service contracted by Banner Health for interpreting to patients, patients' family members and physicians.

Interpretation by patient's family, friends, or untrained staff members is prohibited.

When our medical interpreters are unavailable, Banner Churchill enables physicians and staff to use our Cyracom interpretation phone system. Please ask your nurse to connect you to the Cyracom system. 

Note:  Title VI of the federal Civil Rights Act requires that hospitals provide interpreting services to Limited English Proficient. It is Banner Health policy to use ONLY a hospital medical interpreter or telephone service contracted by Banner Health for interpreting to patients, patients’ family members and physicians. Interpretation by family, friends, or untrained staff members is prohibited.
