Completion of both the Banner Health system and the Ogallala Community Hospital orientation module is required prior to approval of privileges and can be completed anytime while your medical staff application is in process. Our lab provides both inpatient and outpatient laboratory testing. We are committed to serving our patients in the most efficient, caring manner possible.  

Ogallala Community Hospital has selected Horizon Laboratories and Mayo Medical Laboratories as its primary reference laboratories.  Horizon Laboratory is a Banner Health lab that will handle a majority of specimens that need to be sent to a reference laboratory.  We also have access to Mayo Clinic’s extensive menu of laboratory tests and clinical expertise for advanced lab testing. 

Through its worldwide network of more than 4,000 clients, Mayo Medical Laboratories enables hospitals around the world to send tests to the laboratories of Mayo Clinic, which are some of the most sophisticated in the world. Through this arrangement, Ogallala Community Hospital will offer our community immediate access to complex tests performed by Mayo Clinic.  

Outpatient testing is available 7:15 a.m. - 4:30 p.m., Monday - Friday

Paternity testing, breath alcohol testing and drug screen collections available by appointment only (Tuesday - Thursday). Call Central Scheduling for appointments at (308) 284-4011.

Our laboratory staff members are nationally certified as medical laboratory technicians or medical technologists. 

Ogallala Community Hospital Laboratory is committed to comply with the Health Insurance Portability and Accountability Act (HIPAA), Security, and Electronic Transaction Standards.  

The Health Information Management Services department at Ogallala Community Hospital is responsible for keeping complete medical records for each patient.

Obtaining a copy of your Medical Record

Your original medical record is property of Banner Health, but the information in it belongs to you. 

To obtain copies of your medical record information, please print a release form. You may present this completed form:

  • In person in the Medical Records department 
  • By mailing to Medical Records, Ogallala Community Hospital, 2601 N. Spruce St., Ogallala, NE 69153 
  • By faxing to Medical Records at (308) 284-7239

The first 25 pages of your medical record are copied free of charge. A charge of $.50 per page will incur thereafter. 

In order to fulfill your request in a timely manner, please ensure this form is signed and complete before submitting. A photo ID is required to be shown when picking up your copy of your medical record. You may also request your medical record to be copied onto a CD for your convenience.

To protect our patients, staff and visitors, Ogallala Community Hospital has an extensive plan in place for emergency management. 

Regular meetings and drills are conducted to ensure the facility is ready to face any potential internal or external threat.

Codes for overhead pages are posted throughout the building. Becoming familiar with these codes will help you to respond appropriately in the event of an emergency. These instructions are also available as an attachment to your security badge.

Information for Physicians - Frequently Asked Questions

1. What is Emergency Management?
The role of Banner’s Emergency Management department is to help prevent/mitigate, prepare for, respond to, and recover from scheduled and unscheduled events which could adversely affect our staff, facilities or our ability to provide patient care.
  
2. How does Emergency Management work at Banner Ogallala Medical Center?
OCH uses the hospital incident command system (HICS) during all incidents and events. The hospital command center (HCC) is normally in the OCH ED Conference Room unless other wise stated. 

3. What is the role of the physician during a disaster?
Primarily that of treatment in the various treatment areas. 

4. How would the physician be contacted if a disaster occurred?
The medical staff office will contact you in the event of a disaster.

5. What disaster training is available for physicians?
Specific emergency management training is available through the Banner Learning Center. For additional resources contact the Emergency Management Coordinator.

6. How can physicians become more involved with the Emergency Management?
Physicians are encouraged to attend the emergency management meetings and participate in the various exercises.

7. Where can physicians go for more resources about Emergency Management, disaster response and preparedness?
There are numerous resources on family and office preparedness on the Banner website available for download.
 

Ogallala Community Hospital has access to resources to provide interpretation for our patients who have communication barriers such as limited English proficiency or deafness. 

To respect confidentiality and to ensure that the patient receives accurate interpretation, it is Banner’s policy that a patient’s family or friends not be used to interpret. 

To access interpretation services for a patient, please inform the patient's nurse.

OCH Resources for Interpretation:

Video Remote Interpretation (VRI):  Interpreters available on a computer screen equipped with a camera and microphone with technology similar to Skype.  VRI provides interpreters for the following languages: American Sign Language, Spanish, Russian, Vietnamese and Somali. 

Telephone Interpretation: Telephone interpreters available from any telephone in the hospital any time of the day or night for more than 150 languages.
