Concierge Medicine at Banner Executive Health and Wellness is open to individuals and families. Family members in your household can be added to your plan at a lower cost.

All members receive the unique features of Concierge Medicine.

Medical costs, such as the ones described below, are not covered by your membership fee:

• Hospital and/or emergency room
• Medical specialists’ appointments and care
• Prescriptions
• Urgent care
