Each care plan is unique to each patient. The number of in-person visits required will vary based on your individual needs. We understand wound care can seem overwhelming. Our home care team will work with you, your doctor and your caregivers to schedule visits that will optimize your healing and make you feel comfortable and supported.
Our wound care team focuses on education and training for you and your caregivers so you are empowered to care for your own wounds. This includes learning proper cleaning and dressing for existing wounds, as well as addressing any underlying conditions that may be causing your wounds.
We will document your progress from the beginning of treatment through discharge from the program. We use our Home Care Home Base program – our electronic record database – as well as photos to track your wound progress and make sure you are healing. With our Home Care Home Base technology, we will label each wound that needs care and work closely with you to monitor your healing.
