The application criteria for this fellowship include:
- Graduate from Accredited NP/PA training Program
- Pass appropriate certification examination
- AZ Licensure/AZ Licensure-Eligible
- Active BLS & ACLS Certifications
- Eligible for DEA number
- Personal Statement: one-page typed statement on why you are interested in postgraduate surgical training program
- 2 Professional Letters of Recommendations (including one from your graduate program director if you graduated within the last 2 years)
- Curriculum Vitae/Resume
- Official Transcripts from Graduate Program
- Physician Assistants will require official NCCPA Exam scores
- Nurse Practitioners must submit Adult Gerontological Acute Care NP Certification and Registered Nurse First Assist Certification
