A: This incentive offers a $30 reward to Banner Health Plan members who attend their appointment and complete the required screening in 2024, between October 1 and December 31, 2024
Q: Who is eligible?
A: Any eligible Banner Health Plan member who received notification of the reward and completes their exam, between October 1 and December 31, 2024.
Q: Where can the reward be used?
A: The reward is available to be used at Albertsons, Safeway, Kroger, Fry’s, and CVS (with delivery options available with Albertsons and Safeway).
Q: When should the gift card arrive?
A: Please allow 4 – 6 weeks after the date of your exam, for delivery of the gift card.
Q: Are there restrictions with my purchases?
A: This card will not allow the purchase of products such as tobacco, alcohol or firearms.
Q: Is there a limit of gift cards per household?
A: No. Each household member that is eligible for the reward and has completed their exam during the reward time frame will be eligible for the gift card.
Q: Does the gift card expire?
A: The gift card will need to be used within one year of the issue date.
Q: Can the card be used at an ATM?
A: No, this card reward is available to be used at Albertsons, Safeway, Kroger, Fry’s, and CVS.
Q: Can one household member be eligible for multiple rewards?
A: Yes, if the member is eligible for more than one reward.
