Banner – University Family Care members now have access to personalized nutrition support at no cost. Members can meet with a registered dietitian via phone or video calls to create a customized nutrition plan based on health, budget and preferences. Benefits include: 

  • Personalized nutrition guidance from a registered dietician
  • Tips to save on groceries and access to affordable recipes
  • Help signing up for SNAP benefits (if eligible)

Member Outcomes

  • 34% of members diagnosed with obesity achieved 5% weight loss at 1+ years 
  • 49% improved Nutriscore by 5%+
  • 36% of members identified improvement in reported food insecurities
  • 79% of members saw improvement in one blood pressure reading by 5 mm Hg

Assurance Wireless Lifeline Program

Banner – University Family Care members may be eligible for no cost mobile phone service through the Assurance Wireless Lifeline program. This program provides qualifying low-income households with FREE wireless service, so they can stay connected to what matters the most – healthcare, loved ones, government services and more. 

Clinical Care Evaluator (CPR) Transitional Care Program 

Clinical Care Evaluators support adult behavioral health inpatient members in Maricopa County by assessing their needs and connecting them to essential services. The goal is to provide proactive outreach during this transition period, ensuring members receive the necessary support for a smooth recovery. This program helps to address the following service gaps between levels and types of care:

  • Timely accessibility to outpatient services (counseling, medications, etc.)
  • Unmet Social Determinants of Health (SDOH)
  • Limited support post hospital discharge

Member Outcomes 

Members are 5.8% more likely to attend a follow-up appointment within 7 days of hospital discharge and 11% more likely to attend a follow-up appointment within 30 days of hospital discharge. 

Rovicare 

Rovicare is a healthcare coordination platform designed to streamline patient transitions, care management and provider communication. It helps improve efficiency by automating workflows, tracking patient progress and ensuring timely follow-ups. This platform enhances patient outcomes by offering real-time data insights, personalized care plans and secure communication tools. By simplifying care coordination, we can deliver more effective and patient-centered care. 

Incentive Program

Banner Health Plans identified key areas needing improvement to enhance member healthcare outcomes. These focused areas target subpopulations that have experienced healthcare disparities. Healthcare disparities arise when there are significant differences in the availability and accessibility of health services within specific groups. The incentive program aims to boost participation, reduce access to barriers and promote healthcare equity. The identified focus areas include:

  • Oral evaluation/dental services 
  • Colorectal cancer screening 
  • Cervical cancer screening 
  • Breast cancer screening 
  • Diabetes monitoring 

Condition Management Program

This initiative supports members diagnosed with substance use disorders (SUD) who enter Behavioral Health Residential Facilities (BHRFs). The goals of this program are to engage members in their recovery journey by providing relapse prevention education, peer support services, outpatient behavioral health connections and medication-assisted treatment (MAT) or medications for opioid use disorder (MOUD) as needed. This program emphasizes care management engagement and proactive outpatient behavioral health follow-up to reduce hospital readmissions. Additionally, it facilitates a smoother transition from inpatient care, ensuring members receive continued support for long-term recovery and substance use disorder management.

La Frontera Program

This initiative supports family planning, prenatal care and postpartum care by providing accessible information and resources to women of childbearing age who may be experiencing substance use or mental health challenges. 

Back to School Campaign 

This initiative aims to improve access to child and adolescent well-care visits, particularly among populations experiencing health disparities. This program encourages families to prioritize preventative care, ensuring children and adolescents receive vaccinations, developmental screenings and overall health assessments before the school year begins. Targeted outreach efforts focus on communities with historically lower well-care visit rates, addressing barriers such as transportation, healthcare access and awareness. Additionally, it promotes early detection of health concerns, improved long-term outcomes and greater healthcare equity for all children and adolescents.

Tribal Outreach Program

This initiative is dedicated to improving healthcare access and outcomes for American Indian Communities by addressing barriers to preventative care and wellness services. Through culturally sensitive outreach, the program provides education, support and care coordination, ensuring members receive essential health services. The Tribal Care Coordinator works closely with community members to facilitate scheduling, navigate healthcare systems and promote preventative measures like breast cancer screening and well-care visits. This program helps reduce health disparities and support the long-term well-being of tribal populations. 

The Big Pink Bus

The Big Pink Bus is a mobile mammography unit designed to bring breast cancer screenings directly to communities, making early detection more accessible for women, especially those facing barriers to healthcare. This initiative focuses on reaching underserved populations by providing on-site mammograms by eliminating obstacles such as transportations challenges, financial restraints and scheduling difficulties, the Big Pink Bus increases screening rates and early diagnosis, ultimately improving health outcomes.

Text Message Campaigns 

This outreach strategy is designed to remind members about important preventative healthcare services, such as wellness visits, dental exams, immunizations and routine screenings. The campaign helps increase appointment adherence and early detection of health issues. These messages provide scheduling assistance, educational resources and links to support services. By leveraging mobile communication, we aim at reducing gaps in care, improving health outcomes and promoting proactive healthcare management.
